The potential winner need not be present to win. If the potential winner is not present, OIHS will notify him or her of the results of the drawing at the mailing address or email address provided by the potential winner at the time he or she entered the drawing.
  4. The prizes are funded by the raffle proceeds or other third parties.
  5. The winner will be issued any Internal Revenue Service (“IRS”) tax forms or other required documentation and/or reporting forms from OIHS for the prize awarded if required, and the winner is solely and exclusively responsible for all state, federal, or local income taxes that may result from his or her acceptance of prize. In some circumstances, OIHS may be required by law to withhold and remit taxes to the IRS on winner’s behalf a percentage of prize awarded.
  6. The raffle is subject to these rules and all federal, state, and local laws. OIHS shall be entitled to interpret these rules and the laws as needed.
  7. No purchase or contribution is required, but OIHS suggests a minimum donation of $100.00 per raffle ticket.
  8. The raffle is only offered in the State of Florida, and may be void outside the raffle area or where prohibited.
  9. To be eligible, each participant must be at least eighteen (18) years old.
  10. The potential winner must present the original winning raffle ticket to OIHS within five (5) business days of OIHS’s notice to the potential winner of the results of the random drawing. Following submission of the winning raffle ticket, the potential winner will receive a response from an OIHS representative with further instructions on how to present and/or execute all required verification paperwork (as described below). The original winning raffle ticket is the only valid receipt for claiming a prize.

  13. Prize or prize components are not exchangeable or redeemable for cash, may not be sold, bartered, or auctioned, and must be accepted as awarded, without substitutions. The right to receive the prize is not transferrable. Any portion of the prize not accepted by winner is forfeited and no substitute will be offered or permitted. The prize may not be substituted except that OIHS in its discretion may substitute a prize, or portion thereof, with a prize or portion of equal or greater value if it deems necessary. Any such changes will be announced.
  14. The prize offered is provided “as is” with no warranty or guarantee either express or implied by OIHS.
